What’s Included in the SmileCare Plan?
Your membership includes essential preventative care, plus discounts on additional services:
- Two Check-up Exams
- Two Prophy or Periodontal Maintenance Cleanings
- Two In-Office Fluoride Applications
- Routine X-rays Needed for Exams
- 15% OFF Additional Cleanings
- 15% OFF All Other Services
All benefits must be used within the 12-month membership period and do not carry over.

How It Works
Your membership is valid for 12 months from the enrollment date.
It automatically renews each year unless canceled with 30 days’ notice.
Cancel within 30 days if no benefits have been used for a full refund.
Once benefits are used or 30 days pass, the plan is non-refundable.
Payment for any treatment balance is due at time of service.
Exclusions & Limitations
Please note:
- Not valid for patients with dental insurance or other dental coverage
- Only valid for services provided at our office (not with specialists or other clinics)
- Not usable for treatment related to workers’ compensation or third-party liability
- Membership is non-transferable and cannot be combined with other discounts or promotions
